Responsibilities
  • Assess needs for Case Management Services:
  • Determine appropriateness of case management referrals to insure that SMI/SED criteria and needs can be met.
  • Consult with referral source as needed.
  • Inactive/close cases when case management services are no longer needed.
  • Provide direct services to assigned caseload:
  • Conduct home visits with assigned caseload.
  • Conduct face-to-face interviews with clients and significant others to assist with accomplishment of service plan.
  • Identify appropriate community service needs of those persons in need of case management and determine appropriate community resources to meet those needs.
  • Advocate on behalf of the clients with regard to social, economic and residential requirements.
  • Assist the client through crisis situations and/or arranging for the provisions of such assistance by other professionals/personal care givers.
  • Complete Human Service Needs Assessment as required.
  • Maintain client’s files and related paperwork of acceptable quality in a timely manner:
  • Develop written service plans for each client and evaluate and revise plan as needed.
  • Complete progress notes and billing sheets as required.
  • Complete client release of information forms.
  • Provide transportation to clients:
  • Transport only clients who actually need transportation.
  • Ensure that all community resources, such as family, friends or public transportation are utilized.
  • Maintain a valid Alabama Driver’s License.
  • Perform all administration duties as required:
  • Attends and actively participates in In-Service and Quality Review as required.
  • Utilizes supervision/consultation as indicated.
  • Attend staff meetings as required.
  • Attend case management meetings as scheduled.
Prerequisites

Requires Bachelor's Degree in Behavioral/Social Sciences Field and a valid AL Driver's License.
