30-Under the general direction of the ACNO, the Patient Placement Coordinator works closely with Admitting, the Emergency Department (ED), and inpatient nursing leadership to maintain hospital access and assign patients to clinically appropriate beds. The Patient Placement Coordinator plays an active role in managing patient flow. In this capacity, the coordinator manages phone calls related to patient placement and flow and utilizes hospital computer systems to support patient throughput across the organization.
The role collaborates with patients, families, and staff from all admission sources and uses web-based tools to facilitate capacity management and communication among patient placement teams and nursing units. The Patient Placement Coordinator maintains in-depth knowledge of state-mandated rules and regulations. Strong attention to detail and the ability to multitask are essential to ensure accurate and timely patient placement. Responsibilities also include running daily reports to support operational planning and reconciling multiple systems to confirm accurate placement data.
Additionally, the coordinator provides support to the Patient Care Services Scheduling Coordinator as required and will function in this role on off shifts, holidays, and weekends. The Patient Placement Coordinator works in collaboration with patients, families and staff from all sources of patient admissions. This person uses web-based tools to facilitate capacity throughput and communication among colleagues in patient placement and other nursing units.
Maintains an in-depth knowledge of the state mandated rules and regulations governing the preparation of death certificates. Attention to detail, and multi-tasking is essential in order to consistently and appropriately place patients. Runs daily reports for planning of various job functions. Reconciles multiple systems to confirm accurate patient placement. Will also function as support and will assist the Patient Care Services Scheduling Coordinator as needed and will function in this role when needed
30 hours per week - 10 hour shifts
10:00am-8:30pm - Every other Weekend and Every Other Holiday

Clear verbal communication and active listening for patient, provider, and interdepartmental interactions.
Ability to maintain focus and attention to detail in a busy office or clinical environment. Possible exposure to stressful situations involving patients, families, or staff. Infrequent exposure to hospital environmental factors such as proximity to patient care areas, which may involve contact with individuals experiencing illness.
