SummaryProvides personal care, light housekeeping, and assistance with Activities of Daily Living (ADLs) as outlined in each SE participant's care plan in participant homes as well as at the PACE center; exercises independent judgement; reports any changes in participant status to the IDT; participates in infection control precautions and increased cleaning and disinfecting of the PACE center as assigned and in accordance with current CDC guidelines.
Responsibilities- Under the supervision of the RN, assist with ADL needs of participants at the PACE center and in participants' homes (community, assisted living facilities, rest homes, supportive housing).
- Contribute to developing a care plan through interaction with the Interdisciplinary Team.
- Collaborate with team members to assure appropriateness and continuity of care.
- Perform non‑skilled treatments such as vital signs, transfers, toileting, bathing, and dressing at the PACE center and in the community.
- Assist supervisory and recreational staff with individual and group programs, including planning, motivation, participation assistance, and evaluation.
- Assist registered therapists and certified therapy assistants with treatments and assigned activities (positioning, transfers, ambulation, exercises).
- Maintain a clean and safe working/living environment in the PACE center or participants' homes.
- Assist with meal and snack preparation, serving, feeding, and cleanup.
- Use safe techniques in all interactions with participants.
- Provide accurate and timely documentation in the EMR and other systems as required.
- Participate in daily cleaning and disinfecting schedules per CDC guidelines.
- Help distribute work for health aides to ensure timely completion of care and site tasks.
- Participate in regular team meetings for health aides at the respective site.
- Perform all duties in accordance with FH and Summit Elder Care policies and procedures.
Completion of an approved Home Health Aide or Certified Nursing Assistant training program is highly desirable, but equivalent work experience is acceptable.
CertificationCertification as a Home Health Aide or Nursing Assistant is desired.
Reliable transportation required.
Experience- One year of experience working with frail or older adults, preferably in a PACE program. If under one year, completion of Summit Elder Care training prior to independent participant work is required.
- Must complete a standardized set of Summit Elder Care competencies for Health Aides annually.
In accordance with the Massachusetts Wage Transparency Act, the pay range for this position is $20.00 to $24.00 per hour. Final compensation will depend on experience, skills, and fit with the role's responsibilities.
