Ambulatory Clinical Pharmacist Specialist – Endocrinology & Anticoagulation Services

Sturdy Health Medical Group is seeking a dynamic and patient-centered Ambulatory Clinical Pharmacist Specialist – Endocrinology & Anticoagulation Services to join its growing multidisciplinary care team. The specialist will provide advanced ambulatory pharmacy services through collaborative practice agreements and team-based care models to optimize medication therapy and improve patient outcomes. The specialist will function as an embedded member of the Endocrinology, Cardiology, and Primary Care teams and serve as a clinical leader for pharmacist-managed endocrine and anticoagulation services across the medical group.

Working in close partnership with physician leadership, population health team, and other healthcare professionals, the specialist will support evidence-based care, enhance access to clinical pharmacy services, and help develop innovative ambulatory care programs. The position is expected to support the development of sustainable pharmacist-led care models that enhance patient access, provider efficiency, and clinical outcomes.

Education/Training:

  • Doctorate of Pharmacy from an accredited institution required
  • Completion of a PGY1 Pharmacy Practice Residency required, a PGY2 in Ambulatory Care Residency highly recommended

Required

Qualifications and Skills:

  • Active Pharmacist license in the state of Massachusetts
  • Advanced knowledge and understanding of anticoagulation and chronic disease management including diabetes, population health principles, practice models, and the healthcare landscape
  • Experience in quality improvement strategies
  • Demonstrated ability to work on a team and coordinate collaborative efforts
  • Excellent interpersonal, verbal, and written communication skills
  • Strong analytical, problem-solving, and decision-making abilities

Essential Job Functions:

  • Develop, implement, and manage pharmacist-provider collaborative practice models, strategies, and initiatives aimed at improving quality of care and clinical outcomes
  • Co-manage patients via a collaborative practice agreement
  • Collaborate with multidisciplinary teams to design and implement population health programs focusing on chronic disease management, prevention, and wellness
  • Educate patients, healthcare providers, and other staff on medication therapy
  • Develop and sustain an anticoagulation clinic across Sturdy Health Medical Group
  • Serve as a preceptor for trainee pharmacists
  • Lead medication policy and procedure development for ambulatory care practices
  • Participate in the formulation of population health policies, strategies, and guidelines
  • Facilitate medication management services to optimize patient outcomes and reduce potential medication-related issues
  • Ensure medication safety and effectiveness, adhering to local and federal regulations

Physical Demands:

  • Ability to sit, stand, walk, bend, and reach throughout the workday
  • Ability to move between exam rooms, clinical work spaces, and office locations as needed to support patient care and interdisciplinary collaboration
  • Ability to frequently use hands and fingers for computer keyboarding, data entry, documentation, and operation of office equipment
  • Ability to communicate effectively with patients, providers, staff, and external partners in person, by telephone, and through electronic communication platforms
  • Ability to read, analyze, and interpret clinical information, medication records, laboratory results, and other healthcare documentation
  • Ability to maintain visual acuity necessary to review electronic health records, medication information, and written materials
  • Ability to perform prolonged periods of computer and telephone work
  • Ability to occasionally lift, carry, push, or move office supplies, educational materials, or equipment weighing up to 15 pounds
  • Ability to travel between physician practice locations as needed to support patient care and program operations
  • Ability to work in a clinical environment with potential exposure to communicable diseases and other healthcare-related hazards, utilizing appropriate safety precautions and personal protective equipment when required
