Position: Case Manager (QIDP)

Pay Scale: $18 - $20/Hour
Location: Main Campus (primarily), group homes, and other locations
Reports To: Client Care Manager
FLSA Status: Hourly/Non-exempt
Employee Status: Full-Time

Work Schedule: Regular business hours Monday to Friday and other hours intermittently or as assigned
Supervision Given: Residential Direct Support Professionals (DSPs) (functional supervision)
Supervision Received: Client Care Manager(s), Executive Director

NATURE AND SCOPE OF POSITION

A Case Manager, under the direct supervision of a Client Care Manager, is a member of the interdisciplinary team and responsible for holistic programming and individual service plans (ISP) for an assigned caseload for Magnolia residential clients – adults with intellectual and/or developmental disabilities. A Case Manager assists with managing the group home operations and daily living activities of their clients in compliance with their individual ISPs and behavior support plans (BSP), if applicable, and provides functional supervision to assigned DSPs.

PRIMARY QUALIFICATIONS

Education:

  • Bachelor’s Degree (B.A. or B.S.) in a professional category that satisfies the applicable regulations.
Work Experience:
  • At least one year of experience working directly with persons with intellectual disability or other developmental disabilities.
Certification or License:
  • Licensing requirements vary depending on how the Case Manager will be designated (e.g., social worker, human services professional).
  • Current Automotive Insurance Coverage required
  • Attendance at Mandatory Training & In-Services
  • Current CPR certification.

Other Considerations:
  • 21 years of age or older.
  • Valid, unencumbered Louisiana driver’s license.
  • Current automotive insurance coverage with acceptable limits as mandated by Magnolia’s insurance carrier.
  • Satisfactory Motor Vehicle Record.
  • Ability to pass background screening including pre-employment drug test, physical, background check, and other requirements.
  • Ability to be reached for assistance when emergencies arise.
  • Weekend, evening, late-night work, and overtime required at times.
JOB FUNCTIONS

Individual Service Plan (ISP)

  • Develops annual ISPs, as a member of the interdisciplinary team, to address goals and objectives designed to increase independence and self-sufficiency in areas of daily life.
  • Supervises implementation and integrates the various aspects of the ISP.
  • Records the client’s progress as measured by objective indicators.
  • Continuously monitors the ISP and proposes revisions as necessary.
  • Reviews the ISP on a quarterly basis and prepares quarterly status reports on the progress of the client relative to the goals and objectives of the ISP.
Client Care
  • Shows utmost respect for people with disabilities and their families.
  • Uses person-centered planning principles for goal development and service planning, incorporating both what is important to and important for the client.
