Home Health Occupational Therapist Assistant

• Provide therapy services planned, delegated, and supervised by a qualified Occupational Therapist according to the patient's Plan of Treatment
• Assist with vocational and educational programs designed to restore, reinforce, and enhance task performance and promote health and self-sufficiency
• Design and adapt equipment and working or living environments
• Fabricate devices to improve function, independence, and community participation
• Provide therapeutic treatment and patient instruction to improve or restore strength, coordination, range of motion, and function
• Report observations to the Occupational Therapist and/or Clinical Supervisor
• Document observations in patient records and prepare clinical notes and progress reports
• Instruct patients and family members on home programs and adaptive equipment
• Participate in care coordination and discharge planning
• Serve as a resource to other healthcare personnel
• Design community reintegration activities and coordinate plans with the interdisciplinary team

• Current and unrestricted OTA licensure
• Current CPR certification
• Valid driver's license
• Auto insurance
• Reliable transportation
• Minimum six months occupational therapist assistant experience (preferred)
• Home Health experience (preferred)
• Must maintain personal vehicle liability insurance
• Must carry vehicle insurance meeting residing-state minimum limits or specified Humana limits, whichever is higher

Demonstrates expertise in providing therapeutic services under the supervision of an Occupational Therapist, focusing on patient rehabilitation, adaptive equipment design, and community reintegration. Proficient in documenting patient progress and collaborating with interdisciplinary teams to enhance patient outcomes.
