The Infusion Nurse provides primary patient care for assigned patients, performing assessments, care planning, evaluation, and education for patients receiving infusion therapy and/or related nursing services in accordance with the care matrices and best practices in the home or alternate site setting.
Responsibilities- Provide direct care for all assigned patients, including initiation and maintenance of all communications with appropriate care team members (pharmacists, prescribers, dietitians, home patient representatives/care partners, and community resource staff).
- Evaluate and educate patients/care partners to safely administer home infusion therapy and provide additional instruction as indicated.
- Evaluate the home environment where care is to be provided and identify interventions required to render it safe and appropriate.
- Develop, with other team members, the plan of care and evaluate response to therapy with each encounter.
- Manage schedule via company‑approved platform per policy; communicate any scheduling changes promptly.
- Coordinate patient care via the approved company platform.
- Participate in the nurse on‑call rotation to ensure continuous support services.
- Document all encounters, communications, and care activities at the time of service per policy and regulatory requirements.
- Maintain strong knowledge of Soleo therapies, protocols, and clinical standards.
- Ensure timely submission of plans of treatment (POT) and required clinical documents per policy and state guidelines.
- Work with appropriate departments to ensure all required patient documents (consents, POTs) are completed and returned within acceptable time frames.
- Provide administrative oversight, including monitoring workflows, ensuring policy compliance, and managing resources efficiently.
- Remain available for emergencies, responding promptly to incidents during nights, weekends, and holidays as required.
- Monday‑Friday 8:30am‑5pm with a variable schedule that may include days, afternoons, evenings, and weekends as needed; per‑diem 8‑29 hours per week.
- Knowledgeable of IV access devices, including IV lines, accessing ports, and infusion medication administration.
- Able to drive up to 2 hours one direction to see patients; mileage reimbursement and hourly pay offered.
- Licensed as an RN in Alabama in good standing.
- Current RN license by the Alabama Board of Nursing in good standing.
- RN degree; bachelor’s degree in nursing (BSN) from an accredited institution preferred.
- Advanced certification in specialty areas of practice (e.g., CRNI, ONS, IgRN) preferred but not required.
- Minimum 3 years of nursing experience, with a background in infusion therapy preferred.
- Experience in ICU and/or cardiac care preferred.
- Proficient with Microsoft Outlook and Microsoft Office Suite.
- Valid state‑issued driver’s license with no significant restrictions.
