Gentiva Hospice seeks an After hours RN Care Manager to provide high-quality hospice care outside standard hours. You will assess and coordinate patient needs during after-hours shifts, maintain primary clinical oversight, and collaborate with physicians and the IDG for continuity of care.
Responsibilities include on-call visits, assessments, admissions, and care plan development, with emphasis on documentation, regulatory compliance, and compassionate family support.
