Gentiva Hospice is seeking a compassionate Hospice RN / RN Case Manager to deliver skilled nursing care in patients' homes across Alabama. You will assess needs, coordinate with an interdisciplinary team, educate families, and document care plans to ensure comfort and dignity at end of life.
The role requires an active RN license, CPR, and the ability to travel to patient homes. Experience in hospice or palliative care is preferred and a valid driver’s license is necessary.
