Overview
Be the bridge between care and community. As a Community Health Representative in Auburn, you’ll connect patients to the services, support, and education they need to live healthier lives.
Hourly Pay: $24.55-$28.35
Benefits- Employer-paid Health Insurance (medical, dental and vision): 100% for employees and 80% for dependents.
- 17 days PTO per year, 11 Paid Holidays, 8+ Sick Days (1 hour for every 30 hours worked)
- Retirement Plan with 6% match
- Establish trusting relationships with patients and their families while providing general support and encouragement.
- Provide basic health education, informal counseling, support, and services focused on general health maintenance and promotion.
- Helps patients access transportation for appointments by coordinating resources, scheduling rides, and providing reminders as needed.
