Case Facilitator I – Adult
A Case Facilitator I – Adult operates under supervision of the assigned Area Director or designated QBHP/LP. A member of a multidisciplinary treatment team, primary job duties include providing case management and activities of daily living skills to adult consumers. Activities of Daily Living (ADL) – synonymous with term "skill building," refers to the identification and development of skills/abilities related to independent living.
Skill acquisition should be objectively demonstrated (action, behavior). Focus is typically on the present and is interpersonal in nature.
Education:
High school diploma or equivalent.
Experience:
Must meet OBHP requirements = Two years' experience in mental health or community services preferred.
Licenses:
Must possess a valid state issued Driver's License.
Shift: First, Full time, M-F, no holidays or weekends.
Salary based on experience.
