Monday-Friday Schedule No Nights No Weekends Supportive Team Environment Make a Real Difference Every Day

As a Case Manager, you'll be more than a service provider; you'll be a coach, advocate, problem solver, and trusted support person.

You'll:

• Help individuals identify goals and create plans for success
• Connect people with community resources and support services
• Advocate for clients and help remove barriers to independence
• Partner with providers, families, and support networks
• Support skill building, recovery, and personal growth
• Maintain quality documentation and service records
• Celebrate victories, big and small, with the people you support

Have a High School Diploma or equivalent

Hold an MHRT-C or MHRT-C Provisional Certification (or are eligible to obtain MHRT-C)

Have experience in behavioral health, mental health, social services, or human services

Have at least one year of experience supporting individuals with mental illness and/or developmental disabilities

Enjoy building relationships and solving problems

Can work independently while also being a great teammate

Have a valid driver's license and reliable transportation
