Must-Have:
- Experience conducting comprehensive assessments of patients
- All nurse assessors must meet the required qualifications
- Experience in a home setting, including working in the field for a Certified Home Health Agency, Licensed Home Care Service Agency, or Managed Long-Term Care
Description:
Required licensure includes a current RN licensure in New York State.
Candidates must have at least two (2) years of experience in a home setting within the last ten (10) years.
Recent experience should include conducting comprehensive assessments of patients.
Preferred qualifications include experience conducting assessments in UAS-NY, especially in the last 365 days.
Experience caring for a geriatric population and managing chronic diseases in a home setting is also preferred.
Combination of home care experience as a paraprofessional or LPN and RN caring for a minimum of two patients per week is beneficial.
Home care experience with only one patient and case management limited to utilization reviews does not satisfy the required qualifications.
Ability to review health status questions and make evaluations and determinations based on UAS criteria for enrollment is essential.
Candidates must demonstrate knowledge of general professional nursing principles and practices.
Excellent organizational, interpersonal, written, and verbal communication skills are required.
Candidates should be able to communicate effectively with multi-generational consumers.
Sensitivity to the concerns of others is essential.
Ability to perform comfortably in a fast-paced, deadline-oriented work environment is important.
Candidates must be able to execute complex tasks simultaneously and work independently.
Responsibilities include performing initial in-home pre-evaluations of patients.
Evaluations involve assessing health status, strengths, care needs, and preferences to guide individualized long-term care service plans.
Reviewing consumer medical documentation and entering evaluation data into electronic forms is required.
Documentation of concerns, conflicting information, and other issues during the evaluation process is necessary.
Emphasizing continuity of care to reduce fragmentation and gaps in treatment is crucial.
Discussion with consumers on health care options, needed supports, service vendor options, and waiver options is expected.
Initiating communication between eligible consumers and enrollment into managed care health plans is part of the role.
Enhancing communication and collaborative relationships with interdisciplinary care team members to improve care coordination is critical.
Collecting quality review data and required documentation for outcome measurement is necessary.
Identifying opportunities for health promotion and illness prevention is part of the responsibilities.
Maintaining a comprehensive working knowledge of community resources and payor requirements for the target population is essential.
Meeting all established performance standards is expected.
Additional duties may be assigned by the Regional Nurse Manager or project management.
