Advanced Practice Provider (Nurse Practitioner or Physician Assistant)
Auburn Memorial Medical Services, PC is seeking a compassionate, clinically skilled, and patient-focused Advanced Practice Provider (Nurse Practitioner or Physician Assistant) to join our outpatient Pain Management team. This position offers the opportunity to provide comprehensive, evidence-based care to patients experiencing acute, subacute, and chronic pain conditions in a collaborative outpatient practice setting. The APP will work closely with pain management physicians, and other healthcare professionals to evaluate and manage patients with a variety of musculoskeletal, neuropathic, spine-related, and chronic pain conditions.
The ideal candidate will have strong clinical judgment, excellent communication skills, and an interest in developing long-term therapeutic relationships with patients while helping improve their function, quality of life, and overall health outcomes.
Responsibilities and Duties:
- Evaluate patients presenting with acute and chronic pain conditions through comprehensive medical histories, physical examinations, and review of relevant diagnostic studies.
- Develop individualized, evidence-based treatment plans in collaboration with the patient and supervising pain management physician.
- Diagnose and manage common pain conditions, including musculoskeletal pain, spine-related pain, neuropathic pain, arthritis, and other chronic pain syndromes within the APP’s scope of practice.
- Review and interpret laboratory results, imaging studies, prior medical records, and other diagnostic information relevant to the patient’s pain condition.
- Provide education and counseling regarding pain diagnoses, treatment options, medications, risks and benefits of therapy, and realistic expectations for pain management.
- Assist with the evaluation and preparation of patients for interventional pain management procedures and coordinate appropriate follow-up care.
- Perform office-based procedures within the APP’s scope of practice, training, and credentialing, as applicable.
- Coordinate referrals to physical therapy, behavioral health, neurology, orthopedics, neurosurgery, primary care, and other specialists when clinically appropriate.
- Monitor patients longitudinally and modify treatment plans based on clinical response, functional goals, diagnostic findings, and evolving patient needs.
- Communicate effectively with pain management physicians, primary care providers, specialists, and other members of the healthcare team to promote continuity and coordination of care.
- Document patient encounters, assessments, treatment plans, medication management, and follow-up recommendations accurately and timely within the electronic medical record (EMR).
- Respond to patient inquiries and clinical concerns through approved communication channels and determine when physician consultation or an in-person evaluation is appropriate.
- Participate in multidisciplinary case discussions and collaborate with physicians regarding complex or high-risk patients.
- Participate in quality improvement initiatives and maintain compliance with all regulatory, accreditation, controlled-substance, and organizational standards.
- Maintain current knowledge of evidence-based pain management practices, relevant clinical guidelines, and applicable state and federal regulations.
Qualifications:
- Current licensure as a Nurse Practitioner or Physician Assistant in New York State, or eligibility for licensure.
- Graduate of an accredited Nurse Practitioner or Physician Assistant program.
- Prior experience in Pain Management, Primary Care, Internal Medicine, Family Medicine, Orthopedics, Neurology, or a related specialty preferred; experienced APPs and qualified new graduates with a strong interest in pain management are encouraged to apply.
- Basic Life Support (BLS) certifications are preferred.
- Proficiency with electronic medical records (EMR).
- Strong clinical judgment, communication, and organizational skills.
