Position Summary:
The Quality Improvement Coordinator is the member of the health center leadership team responsible for assuring that patients consistently receive high-quality and equitable healthcare services in alignment with organizational population health goals. The Quality Improvement Coordinator is responsible for driving improvement projects that lead to better patient health outcomes. This position reports directly to the Business Director at a health centers but has a matrix relationship with Population Health leadership.
Compensation is dependent on skills and experience.
Your contribution to the team includes:
- Serve as a member of health center leadership team as a quality improvement and population health expert and champion.
- Work closely with Population Health leadership in a matrix management relationship.
- Understand and inform organizational quality goals and approaches.
- Analyze, understand, and improve healthcare systems and health outcomes.
- Use a quality improvement methodology like Institute for Healthcare Improvement Model or Lean to bring structure to improvement work.
- Interface directly with health care providers and team members to drive systems improvement.
- Report on progress toward quality goals at both the health center and organizational level via presentations, publications, and conversations.
- Facilitate meetings and discussions around healthcare quality.
- Participate and serve as a member of a larger network of quality improvement experts across the organization to collect successful interventions and work toward developing approaches that inform standard workflows across the organization.
- Serve as the health center expert in quality related data collection, analysis and turning data into information for action.
- Support health center staff to access population health technology and information and coordinate services for high-risk patients.
- Serve as health center coach for healthcare improvement and process improvement methodologies.
- Support quality improvement projects as needed
- Stay up to date on current thinking related to health care quality, health care disparities, and health equity issues by attending webinars, community meetings and other conferences as identified by business directors.
- Attend staff meetings, in-service meetings and participate in agency committees and task force activities providing expertise in the area of quality improvement as needed.
- Maintain good attendance, be punctual and work full scheduled shifts as a condition of employment

Must have’s you’ll need to be successful:
- Bachelor’s degree (BA or BS) from four-year college or university in public health, health administration, or related field and master’s degree preferred.
- A minimum of six (6) months of applied healthcare quality improvement experience.
- Equivalent education and experience accepted.
