Job Description:

The Social Worker Case Manager is responsible for acute care case management, behavioral health assessment, and treatment planning for adult inpatient populations. This role involves safety planning, discharge planning, and support for a variety of patient settings including ICU, PCU, Med Surg, ER, and others.

Responsibilities:

• Perform behavioral health assessments and create treatment plans.
• Manage discharge planning for post-acute placements, including understanding SNF level of care and regulatory requirements.
• Conduct safety planning and apply Involuntary Treatment Act processes when necessary.
• Utilize EMR systems, with EPIC being preferred.

Requirements:

• 4 years of experience with at least 2 years recent acute inpatient.
• MSW, WA state LICSW preferred; LSWAIC considered with supervision.
• Experience in acute care case management and familiarity with involuntary admits processes.
