The Social Worker MSW supports participants and their families with personal and environmental challenges that may impair health or ability to remain in their home environment. In this role you will use strong analysis, interpretation, and interpersonal skills to help patients navigate complex issues.
Responsibilities- Administer assessment tools and gather data; determine the need for optional services.
- Interview families to identify the social work needs of the participant.
- Contact community resources to coordinate services.
- Provide initial and ongoing assessment of the participant’s social, emotional, and cognitive status and needs, involving the participant and other staff in the assessment process.
- Maintain accurate documentation for all assessments, treatment plans, and evaluations.
- Master’s Degree in Social Work; licensure as LSWAIC, LMHC, LMFT, LASW, LICSW (as required by department).
- Experience in a hospital/acute setting or equivalent social service agency, within the medical social work function.
Pay scale: $39.44 – $62.55 per hour (compensation varies by skill set, experience, and certification).
