Position: Case Manager II /Case Manager Trainee - COPES Program

The Case Manager II/Trainee- COPES provides clinical case management services to adults receiving long‑term term services and supports through an Aging and Long‑Term Care of Eastern Washington (ALTCEW) program. Evaluate client strengths and needs, complete a comprehensive assessment, determine level of functioning, develop and implement service plans that provide a reliable and safe plan of care that enables clients to achieve a maximum level of health and independence and targets case stabilization, ensuring independence.

Complete administrative duties and meet required program deadlines. Coordinate with and perform advocacy with relevant parties and community agencies.

ESSENTIAL DUTIES AND RESPONSIBILITIES includes but is not limited to the following:
  • Adhere to program standards and guidelines and follow case management program protocols.
  • Provide long‑term care services responsive to the needs of elderly and adults with disabilities who are eligible for Title XIX Program services.
  • Conduct face‑to‑face comprehensive assessments at the client’s residence to determine level of care, including determining nursing facility level of care needs.
  • Develop ongoing care plans which provide the support necessary to allow clients to live independently.
  • Facilitate the coordination of in‑home caregivers (Individual Providers and Agency Providers) for client care needs.
  • Coordinate with ALTCEW and provide advocacy, coordination, consultation and support to clients, providers and stakeholders across multiple disciplines and systems.
  • Perform program charting requirements, correspondence, and other paperwork accurately and in accordance to program deadlines.
  • Authorize service payment through Provider One/Individual Provider One.
  • Conduct interdisciplinary case reviews and consultations.
  • Establish and maintain a good working relationship with FBH staff and community partners.
  • Perform other related duties as required.
  • Comply with all State WAC requirements and FBH policies and procedures.
  • Requires travel to see clients in the community and travel between work sites.
  • Other duties as assigned.
QUALIFIED CANDIDATES WILL HAVE:

REQUIRED:

High School diploma or its equivalent and one year paid full time on‑the‑job social service experience or one year full‑time equivalent verifiable hours through an internship or volunteer work in social services. Part‑time hours may be considered if hours total one year full‑time equivalent.
