Center Well Home Health seeks a Speech Language Pathologist to deliver in-home therapy, evaluating and treating communication, voice and swallowing disorders, and supporting plans of care. You will educate patients and families, coordinate care, and maintain detailed documentation for reimbursement and quality outcomes.
Ideal candidates have at least six months of HHC experience, a valid license and CPR. A valid driver's license and reliable transportation are required to travel to patient homes.
