Key Responsibilities:

* Complete comprehensive in-home patient assessments
* Develop care plans under primary care providers' orders
* Provide direct patient care per the plan of care and scope of practice
* Ensure accurate and timely documentation
* Participate in QAPI program for top-notch patient care
* Coordinate interdisciplinary home health services

Qualifications:

* Registered Nurse (RN) licensure
* Home Health and/or Hospice experience preferred
* Current BLS certification from AHA
* Licensed driver with an insured automobile in good working order
