Rock Creek Care Center - 260 Racetrack St, Auburn, CA 95603
Salary Range: $130,000 - $160,000 annually

Rock Creek Care Center is seeking an experienced and motivated MDS Nurse / Case Manager to join our dedicated and collaborative team. This key clinical leadership role is for a detail-oriented professional who thrives in a fast-paced skilled nursing environment and is passionate about delivering high-quality, resident-centered care.
Key Responsibilities- Oversee and complete MDS assessments accurately and timely in compliance with state and federal regulations
- Coordinate admissions, discharges, and care transitions
- Communicate regularly with residents and families regarding care plans, therapy services, and progress
- Monitor resident status and adjust care plans based on changing needs
- Perform utilization review to ensure appropriate, timely, and cost-effective care
- Coordinate with interdisciplinary teams, insurance providers, and community resources
- Maintain accurate, thorough documentation and charting
- Support quality assurance initiatives and regulatory surveys
- Provide direct nursing care and clinical oversight as needed
- MDS experience required
- Bachelor's Degree in Nursing or Social Work preferred
- Minimum two (2) years of clinical nursing experience preferred
- Knowledge of Medicare, Medi-Cal, and Medicaid programs
- PCC experience preferred
- Strong organizational, communication, and time-management skills
- Must maintain licensure and remain in good standing with the Department of Public Health
