As the Resident Care Manager, you will be responsible for overseeing the delivery of resident care services, ensuring compliance with regulatory standards, and promoting quality outcomes. You will work closely with interdisciplinary teams to develop and implement individualized care plans, coordinate resident care activities, and provide clinical leadership to nursing staff. The ideal candidate will have strong clinical skills, excellent communication abilities, and a passion for enhancing the lives of residents in a skilled nursing setting.

- Coordinate and oversee the assessment, planning, implementation, and evaluation of resident care plans in collaboration with interdisciplinary teams.
- Conduct comprehensive assessments of resident needs and develop individualized care plans based on resident preferences, goals, and clinical conditions.
- Monitor resident progress and modify care plans as needed to ensure optimal outcomes and quality of life.
- Provide clinical guidance and support to nursing staff, including coaching, mentoring, and ongoing education to promote professional growth and competency.
- Collaborate with physicians, therapists, social workers, and other healthcare professionals to coordinate resident care services and address complex medical and psychosocial needs.
- Ensure compliance with regulatory requirements, including federal, state, and local regulations, as well as facility policies and procedures.
- Participate in quality improvement initiatives, resident care conferences, and interdisciplinary team meetings to promote continuous improvement in resident care delivery.
- Serve as a liaison between residents, families, and healthcare providers to facilitate communication, address concerns, and promote resident‑centered care.
- Current Registered Nurse (RN) license in Washington State or out‑of‑state MSL
- If MSL, follow the Washington Nurse Practice Act.
- If MSL, complete Suicide Prevention Training in advance of hire per state regulations.
- If MSL, Washington State Board of Nursing (WABON) MSL demographic survey.
- Minimum of two years of clinical experience in a skilled nursing or long‑term care setting.
- Previous experience in a leadership or management role preferred, with demonstrated ability to supervise and direct nursing staff.
- Strong assessment and critical thinking skills, with the ability to develop and implement individualized care plans based on resident needs and preferences.
- Excellent communication and interpersonal skills, with the ability to effectively communicate with residents, families, and interdisciplinary team members.
- Knowledge of regulatory requirements and compliance standards relevant to skilled nursing facilities, including Medicare, Medicaid, and survey regulations.
- Commitment to resident‑centered care, dignity, and respect for residents and their families.
- Proficiency in using electronic health record (EHR) systems and other healthcare technology platforms.
- Ability to give direct patient care, including participating in lifting, transferring, positioning, and assistance of residents with walking or in wheelchairs.
- Able to lift 50lbs on a regular basis.
- Ability to use a computer keyboard or touch screen to document resident records.
- Apply your physical and muscle strength to lift, push, pull or carry objects.
- Ability to utilize your lower back and abdominal muscles to support part of the body repeatedly and continuously over time without fatiguing.
- Ability to listen to and comprehend information and ideas provided through conversation, email, and text.
