You will evaluate eligibility, review histories, coordinate with the care team, and provide education and emotional support to patients and families in homes, adult family homes, assisted living, and SNFs across Pierce, Thurston, Kitsap, and King counties.
The role requires an active WA RN license with BLS, strong EMR skills, and the ability to work
