The WISe Care Coordinator coordinates and facilitates the strengths-based and family-driven Wraparound with Intensive Services (WISe) process for trauma-impacted children and families. This position is the care coordinator and facilitator of the multidisciplinary Child and Family Team and is responsible for leading the team through the phases and activities of WISe. The Care Coordinator contributes knowledge and skills to ensure the team process honors each member’s role, responsibility, and perspective.

Work is guided by high fidelity wraparound principles and provided in a team framework, in close collaboration with Akin’s peer partners, therapists, and natural and community supports. Emphasis is on partnering actively with families, natural supports, and other community service providers to promote safe, stable, and healthy families.

This position has access to protected healthcare information (PHI) or confidential identifiable information (CII), both paper and electronic, as necessary, to perform related job duties and responsibilities.

Essential Functions Child & Family Team Facilitation
  • Prepares for and facilitates Child and Family Team (CFT) meetings, including agenda, visual aids, tools, and resources
  • Schedule times and locations convenient for family and team members
  • Serve as the central point of communication for the Child and Family Team process
  • Ensures all team members receive updated copies of the Cross-System Care Plan, documentation of progress, CFT notes within 7 days after each meeting
  • Maintains team focus on progress toward established goals and transition out of WISe.
Family-Centered Support
  • Assists families in identifying their children’s strengths and needs to shape an individualized family vision
  • Completes weekly check-in calls with families and maintain complete documentation of the child’s case records in the mandatory 48-hour window.
  • Ensures respect for the input, needs, and feelings of the child and family
  • Inform families of their rights; obtain necessary consents and releases of information
  • Acknowledge and celebrate milestones, successes, and transitions
Community & Resource Coordination
  • Works with Family Partner to identify family support or other formal and informal resources to support child and family integration
  • Guide each team member to identify priority concerns, minimize conflict, and build consensus
  • Participate in rotating after-hours crisis on-call coverage, providing timely response and support to youth and families outside of regular business hours
  • Complete, update, and communicate the Family Safety and Crisis Plan with family and team members
Care Coordination & System Navigation
  • Maintain deep familiarity with local and statewide resources across child welfare, developmental disabilities, education, and juvenile justice; actively help youth and families understand and access those services
  • Discern which parts of the system and available resources are most relevant to each youth and family’s unique strengths and needs
  • Nurture positive, ongoing relationships with system partners and integrate them into the WISe team based on youth and family voice
  • Establish and maintain a strong connection with each youth's Managed Care Organization (MCO) to ensure access to appropriate Medicaid state plan services
  • Collaborate with all WISe team members to weave their areas of expertise into the planning process, supporting an integrated team approach
Compliance & Documentation
  • Maintains documentation of child’s case records and electronic health record (EHR) according to Akin policy, County and State requirements, and Wraparound fidelity guidelines. Perform reviews of documentation when required by WISe Coach or when need arises (self-audit)
  • Ensures compliance with all Akin policies and procedures. Adhere to all related contracts, Federal, State, and local requirements, and guidelines
  • Maintain productivity standards and ensure timely collection of statistical information for funding and program evaluation
  • Perform self-audits and documentation reviews as required by WISe supervisor and/or manager
