Virtua Hospice is seeking a compassionate Hospice RN Case Manager to deliver end‑of‑life care in New Jersey. You will assess patients in their homes, coordinate with physicians and the interdisciplinary team, educate families on symptom management, and develop individualized care plans.
The role requires 2–3 years hospice nursing experience, NJ RN license, CPR/BLS, and a valid driver’s license. On‑call, weekend and holiday coverage may be required, with statewide travel to patient homes.
