Occupational Therapist – Conyers, GA

FOX Rehabilitation is growing in Conyers, GA, and we’re looking for licensed Occupational Therapists to deliver one‑on‑one care to older adults across private residences and senior living communities through our Geriatric House Calls model.

• Provide one‑on‑one occupational therapy to older adults in private residences, senior living communities, independent living, assisted living, and memory care settings.
• Perform evaluations, develop individualized plans of care, and deliver evidence‑based treatment.
• Deliver therapy sessions that support patient wellness, longevity, mobility, and functional independence.
• Document effectively within FOX’s EMR system in compliance with regulatory standards.
• Collaborate with an interdisciplinary team of PTs, SLPs, and fitness specialists.
• Educate patients, caregivers, and senior living staff on safety, function, and aging in place.
• Provide telehealth or virtual care when appropriate.

• Valid Occupational Therapy license in the state(s) of practice, or eligibility to obtain one.
• Degree from an accredited Occupational Therapy program.
• Basic computer and EMR proficiency.
• Current CPR certification.

• Ability to lift 25–50 lbs occasionally, 10–25 lbs frequently, and up to 10 lbs constantly.
• Perform CPR and provide moderate to maximum assistance with patient transfers and bed mobility.
• Reposition patients, stand/walk for extended periods, climb stairs, and perform therapeutic activities.
• Drive to patient homes and senior living communities as part of the Geriatric House Calls model.
• Wearing PPE when required.

• PRN / FOX Flex Occupational Therapist – Home & Community Visits (pay per unit compensation).
