Center Well Home Health is seeking an Occupational Therapy Assistant to join our home-based care team. You will provide rehabilitative services under the supervision of a licensed OT, help patients regain strength and independence, and support home exercise programs for physical reconditioning and functional recovery.
Responsibilities include documenting patient progress, coordinating with the interdisciplinary team, and educating patients and families on home programs and adaptive equipment.
