Job Summary:

Provides spiritual support and comfort for participants, families, and caregivers of STLC. Specifications

Education :
High School Degree or equivalent. Undergraduate degree in related studies, master's degree of Divinity preferred.

Experience :
At least 1 years experience working with the frail elderly population.

Licensure, Registry or Certification Required :
Valid NC Driver’s License with good driving record.

Special Training :
Meet a standardized set of competencies for the specific position description established by Senior TLC, Inc. and approved by CMS before working independently.

Immunizations: Be medically cleared for communicable diseases and have all immunizations up to date before engaging in direct participant contact

Neonate/Infant Early Childhood Adolescent Adult Geriatric All Age Groups

Key Responsibilities :

  • Serves as Chaplain to Senior TLC providing direct spiritual support and end of life counsel to participants and families in keeping with their spiritual beliefs.
  • Maintains proper documentation of pastoral care visits to participants and their families in the Electronic Medical Record (EMR).
  • Coordinates plan of care along with IDT by working to anticipate and meet participant needs and addressing specific goals.
  • Works closely with IDT and related disciplines to ensure professional service delivery.
  • Provides prayer, sacraments, religious rites, scripture and other religious literature, funeral planning, education, referrals, as well as requested ceremonies such as funerals and memorial services and other activities that may be implemented as interventions.
  • Consults with the participant, caregivers, IDT, and facility staff when initiating care and when significant changes in care needs occur. Reliably communicate pertinent information to appropriate members of the interdisciplinary team.
  • Offers religious and spiritual guidance to participants, caregivers, and staff as requested.
  • As requested, completes assessments to determine participants' values, belief systems and spiritual needs.
  • Facilitates spiritual care groups and participates in End-of-Life Committee meetings. Assist with other committees as needed.
  • Offers bereavement support and grief care to participants, caregivers and families as needed.
  • Supports Senior TLC’s mission to encourage and support the quality of life of seniors wishing to continue living in the community; its vision to be the preferred provider of individualized care for seniors in the community; and its values of respect, integrity, accountability, compatible goals, and compassionate care.
