As a Part-time Hospice Social Worker our role will be utilized as one of three foundational pillars in the patient's overall plan of care, managing all psycho social aspects of the hospice journey! Tapestry Hospice is searching for an experienced Hospice Social Worker to provide both emotional and practical support to patients and families facing serious illness or end-of-life issues. This position works closely with members of the Interdisciplinary Team as well as with your patients and their loved ones during the hospice experience and beyond.
Qualifications:- Licensed as a Social Worker through the Board of Georgia
- Possess effective communication skills (including strong verbal and written)
- Possesses a history of building and influencing patient and family relationships
- Holds a current driver's license, car insurance, CPR certification, and positive driving record
- Hospital, home health, or hospice experience is extremely preferred
- Part-time availability
- Assess patient/family social and emotional factors to estimate their potential to cope with terminal illness and death.
- Provide psycho social counseling and support to the patient/family experiencing emotional, social, and economic conflict.
- Participate in Interdisciplinary Team (IDT) meetings to establish, review, and revise the plan of care for each patient, including exchange of information, review of problems, and sharing of professional support.
- Maintain the dignity of the dying patient.
