Job Summary:
Under limited supervision by the Regional Services Administrator, the Case Expeditor coordinates and facilitates timely access to appropriate community-based services for individuals transitioning from hospitals and other institutional settings. The position collaborates with individuals, families, hospitals, providers, courts, law enforcement, and community partners to support successful transitions, continuity of care, and sustained community living.
The Case Expeditor serves as a regional resource by providing consultation, technical assistance, education, and service coordination in accordance with DBHDD policies, procedures, and service guidelines.
Key Responsibilities
• Collaborate with Community stakeholders and as directed may represent the region in community meetings
• Coordinate transition and discharge planning among individuals, hospitals, providers, families, and community partners.
• Provide customer service duties including descriptions of services, explanation of the State and Regional system and handling customer complaints
• Facilitate timely access to recommended services and support.
• Coordinate linkages to appropriate services before, during, and after discharge.
• Collaborate with state hospitals and community providers to implement Transition Action Plans and address service needs and/or gaps.
• Monitor and track individuals transitioning from hospitals and other institutional settings and identify barriers to successful community placement.
• Develop solutions with providers and community partners to promote continuity of care and community integration.
• Maintain effective relationships with provider agencies and remain informed of service capacity, individual needs, barriers, and available community resources.
• Provide consultation and technical assistance to providers, hospitals, and other stakeholders.
• Support the development and delivery of training and educational activities related to transition planning and community-based services.
• Promote person-centered planning and meaningful collaboration with individuals, families, providers, and community partners.
• Maintain accurate documentation, tracking systems, reports, and outcome data related to case expediting and transition activities.
• Perform other duties as assigned in support of regional and departmental priorities.
Preferred Qualifications
• Master’s degree in a related field and two years of related experience
• Experience working with individuals with significant behavioral health or developmental disability needs.
• Knowledge of DBHDD services, policies, procedures, and community-based service systems.
• Experience collaborating with behavioral health providers, hospitals, courts, law enforcement, and community organizations.
• Experience providing training, technical assistance, consultation, or stakeholder education.
• Strong communication, organizational, problem-solving, and interpersonal skills.
