Nurse Case Manager Senior - Field Nurse
Work location:
This field-based role is located in the Hall County, GA area. Associates will primarily operate in the field, traveling to client sites or designated locations as required, with occasional office attendance for meetings or training. This approach ensures flexibility, responsiveness, and direct engagement. The position will include the following GA counties:
Hall, Gwinnett, Habersham, Banks, Jackson, and Forsyth. Candidates must be within a reasonable commuting distance from the posting location(s) unless an accommodation is granted per law.
Work schedule:
Monday - Friday 8:00am to 5:00pm EST with a late evening shift 11:00am to 7:30pm EST.
Assessment:
Requires an online pre‑employment skills assessment, free of charge. Candidates meeting the minimum requirements will be contacted with instructions. Must complete the assessment within 48 hours of receipt.
- Provide care management for members with complex and chronic needs by assessing, developing, implementing, coordinating, monitoring, and evaluating care plans to optimize health care across the continuum.
- Conduct assessments to identify individual needs and develop specific care management plans.
- Implement care plans by facilitating authorizations/referrals within benefits structures or through extra‑contractual arrangements.
- Coordinate internal and external resources to meet identified needs.
- Monitor and evaluate effectiveness of care plans and modify as necessary.
- Assist in problem‑solving with providers, claims, or service issues.
- Coordinate referrals to local and statewide resources including behavioral health, housing, transportation, and food assistance.
- Partner with community health organizations, advocacy groups, and outreach teams.
- Plan, coordinate, and deliver educational events in collaboration with community partners, employers, or local health organizations.
- Provide group‑based and one‑on‑one education on chronic conditions, medication adherence, preventive screenings, nutrition, and self‑care.
- Support initiatives addressing health equity and culturally responsive care.
- Assist in developing utilization/care‑management policies and procedures.
- Chair and schedule meetings; present cases at Grand Rounds/Care Conferences and participate in interdepartmental and cross‑brand work groups.
- Act as a preceptor for new care‑management staff and develop focused skill set in specific disease processes or traumatic injury.
- Participate in department audit activities.
- Bachelor’s degree (BA/BS) in a health‑related field and minimum of 5 years clinical experience, or equivalent combination of education and experience.
- Current, unrestricted RN license in applicable state(s).
- Nursing experience in Home Health, Managed Care, Case Management, or Care Coordination.
- Case Management certification.
- Strong communication and presentation skills.
