MDS LPN Responsibilities

• Coordinate the completion of Minimum Data Set (MDS) assessments for all residents in accordance with regulations
• Ensure accuracy and timeliness of MDS assessments and care plans
• Collaborate with interdisciplinary teams to develop individualized care plans
• Monitor and update resident care plans as needed
• Communicate effectively with staff, residents, and families regarding MDS assessments and care planning
• Stay up-to-date on regulations and guidelines related to MDS assessments
• Participate in quality improvement initiatives related to resident care

• Current LPN license in the state of Florida
• Minimum of 2 years of experience in MDS coordination in a long-term care facility
• Strong knowledge of MDS assessment process and regulations
• Excellent communication and interpersonal skills
• Ability to work effectively in a fast-paced environment
• Detail-oriented with strong organizational skills

Background screening requirements:
Must be able to pass level 2 fingerprinting with an Eligible status.
