Under supervision, provide professional nursing care in accordance with nurse protocols, policies and procedures. Focus on prevention of illness, injury or disability, promotion of health and maintenance of health of populations. Provide outreach, education, and case management to high‑risk perinatal & postpartum women and infants up to one year of age through home visits. Conduct home visits for prenatal, postpartum and infant assessments and coordinate with providers.
Educate patients, offer referrals and document observations. Attend required meetings. May supervise additional staff. Willing to travel within District 2 as necessary. Familiar with OB/GYN hospital practices.
Associate’s degree in nursing from an accredited college or university AND completion of a PHN course within one year of hire AND current Georgia Registered Professional Nurse license AND CPR certification. No experience required.
PH RN 2Bachelor’s degree in Nursing from an accredited college or university AND current Georgia license to practice as a Registered Professional Nurse AND CPR certification OR Associate’s degree/diploma in Nursing from an accredited college AND one year of PHN experience AND completion of a PHN course within one year of hire AND current Georgia Registered Professional Nurse license AND CPR certification.

- Excellent written and oral communication skills
- Ability to communicate effectively with corporate clients
- Self‑directed & self‑motivated
- Ability to complete tasks without detailed supervision
- Excellent computer skills
- Ability to read, write and speak Spanish
