Position: Licensed Practical Nurse-Part-Time (6am-6pm) Monday & Friday

LPN Charge Nurse (Part‑time, Monday & Friday, 6am‑6pm)

Qualifications
  • Graduate of an accredited school of nursing or a vocational school of practical nursing.
  • One to two years of experience preferred.
  • Good communication, comprehension, and interpersonal skills.
  • Working knowledge of Windows, Microsoft Office Suite, and the Internet.
  • Current LPN license.
Essential Functions
  • Supervise, provide, and direct nursing care in compliance with facility policies and procedures.
  • Make observations on an advanced level; report and record observations appropriately; form nursing judgments and carry out nursing actions.
  • Follow established standards of nursing practice.
  • Assume responsibility for compliance with federal, state, and local regulations; interpret policies for staff, families, and physicians.
  • Perform daily rounds, develop and implement accurate comprehensive care plans based on each resident’s needs.
  • Follow policies for medication passing; order, credit, and keep accurate accounting of all medications (stock, unit dose, narcotics).
  • Follow staffing guidelines and make nursing assignments; provide accurate reports to maintain continuity of care.
  • Monitor diets, supervise meal trays and snacks; assist in passing meals and feeding.
  • Follow admission procedures and communicate admissions to other departments, staff, physicians, and service providers.
  • Ensure employees follow chain of command and personnel policies.
  • Assign nursing assistants and ensure efficient completion of assignments; supervise, document, and correct performance; report issues to Unit Managers/Director of Nursing.
  • Participate in performance evaluations of nursing assistants; provide counseling, coaching, and training; present evaluations to staff.
  • Communicate professionally; follow written and verbal directions; provide exemplary customer service.
  • Report changes in physical or mental conditions promptly; follow company, facility, and state policies and procedures.
  • Attend staff meetings, in‑services, interdisciplinary team, and committee meetings as assigned.
  • Observe environmental conditions; cooperate with other staff to ensure quality environmental conditions.
  • Document resident information timely, accurately, and confidentially per procedures; document changes and notify POA and doctor; perform comprehensive assessments.
  • Verify and update MAR/TAR records monthly.
  • Adhere to resident rights documentation; know resident rights and help residents exercise/protect them; report complaints and maintain confidentiality.
  • Adhere to safety & compliance policies; ensure residents safe; demonstrate proper use of equipment; report equipment needs or repairs.
  • Use protective equipment; follow infection control protocols; practice universal precautions and life safety measures.
