• Provide patient-centered palliative care to adults with advanced, life-limiting, progressive and terminal illness.
• Provide initial and ongoing clinical assessment for patients with advanced and/or progressive medical conditions.
• Assess patients’ needs, establish goals of care and recommend treatments and therapies that manage symptoms.
• Collaborate with primary care provider, specialists, Population Health, Transitional Care Management, Chronic Care Management, patients and their families to manage palliative care needs, including emotional, spiritual, coordination of care and bereavement support.
• Facilitate referrals for additional care services when appropriate.
• Prescribe medications and order diagnostic tests.
• Maintain timely, accurate clinical documentation in compliance with professional standards.
• Maintain a safe and clean working environment.
• Protect patients and employees by adhering to infection‑control policies and protocols.
• Comply with federal, state and local legal and professional requirements.
• Maintain professional and technical knowledge by attending educational workshops, reviewing professional publications and participating in professional societies.

Education: Master’s degree from an accredited Nurse Practitioner / Physician Assistant program.

Licensure or Certification:

• Current unencumbered license to practice as a Certified Nurse Practitioner (APRN) or Physician Assistant (PA‑C).
• Valid driver's license and current automobile insurance coverage.

Experience: Three years of experience as a Family Medicine NP / PA. Experience in hospice or palliative care setting preferred.

  On‑site and home visits within a 30‑mile radius.
