Nurse Navigator - Oncology Clinic

Role: The Nurse Navigator (NN) is a licensed registered nurse specializing in disease-specific care, providing individualized support to patients, families, and caregivers to facilitate timely access and adherence to treatment plans.

Responsibilities: Serve as a subject matter expert, coordinate patient care across multiple settings, perform the nursing process, promote patient-centered care, and support program growth. Collaborate with interdisciplinary teams, participate in performance improvement, mentor colleagues, and stay updated on advancements in oncology care. Ensure compliance with organizational policies and standards, and contribute to data collection and evidence-based practice.

Qualifications: Minimum of 3 years nursing experience, with 2 years in disease-specific or outpatient care preferred. Requires RN licensure, BLS certification, and strong critical thinking, time management, and emergency response skills. Associate or Bachelor's degree in nursing is preferred.
