Regency Southern Care Hospice is seeking a dedicated Hospice RN / RN Case Manager to lead in-home patient care. You will assess patients, administer nursing interventions, and coordinate with an interdisciplinary team to build personalized care plans that support families and end-of-life goals.
Required is an active RN license with CPR certification and at least 1 year of RN experience; hospice experience is preferred.
