Piedmont Healthcare is seeking a Patient Care Manager to lead compassionate hospice services within our Healthcare & Medical Services team. In this role, you will oversee interdisciplinary hospice care, ensuring high-quality, patient- and family-centered services that align with regulatory standards and best practices. You will mentor and support clinical staff, coordinate care plans, monitor performance metrics, and collaborate across teams to drive continuous improvement.

Responsibilities
- Oversee daily hospice operations and interdisciplinary team coordination across home and inpatient settings
- Supervise, mentor, and evaluate hospice nurses, social workers, chaplains, CNAs, and other team members
- Ensure development, implementation, and review of individualized hospice care plans for patients and families
- Coordinate patient admissions, transitions of care, and timely scheduling of visits and services
- Collaborate closely with physicians, hospital staff, and community partners to support appropriate hospice referrals and continuity of care
- Monitor clinical quality indicators, regulatory compliance, and documentation standards; implement quality and process improvement initiatives
- Support symptom management oversight and escalation of complex clinical cases to appropriate providers
- Manage staffing levels, schedules, and workload distribution to ensure safe, effective patient care
- Provide education to staff, patients, families, and community partners on hospice philosophy, services, and eligibility
- Participate in budgeting, resource allocation, and cost-effective use of staff and supplies
- Promote a culture of respect, inclusion, and patient-centered care aligned with Piedmont Healthcare's values
- Support ongoing staff training, competency validation, and professional development initiatives
