Job Summary

The Care Partner is a support care provider who functions under the direction of the Registered Nurse. The Care Partner’s primary focus is to promote a positive patient experience by partnering with the nurse, physician, health care team, and other health care providers to provide excellence in patient experience and to assist with daily living activities and comfort measures. The Care Partner assists other health care providers with safe, age‑appropriate and culturally aware care by performing basic patient care functions such as ADLs, ambulation, and vital signs.

The Care Partner may also perform advanced technical tasks, including phlebotomy, simple treatments, dressing changes, oral suctioning, and discontinuing Foley catheters. Additional clerical functions such as data retrieval, unit supplies management, and transporting supplies or patient medications are required. The Care Partner must be able to work in a fast‑paced environment that requires multi‑tasking and effective communication with the Registered Nurse and ancillary team, perform under stressful conditions with a compassionate, respectful, and service‑mindset, take direction from a variety of care providers, and be organized with excellent communication skills.

• Provide excellence in patient experience by partnering with the health care team, patient, and family.
• Support an environment of healing or wellness for patients: address patient needs, follow the plan of care as assigned, engage patient and family in compliance with the treatment plan, and contribute to the plan of care or treatment based on assigned tasks and patient/family input.
• Implement safety interventions to minimize risk, protect patient rights and dignity, maintain confidentiality in accordance with HIPAA regulations, and participate in hourly patient rounding.
• Answer telephone and call lights promptly and professionally, notify the appropriate individual, and resolve patient concerns or escalates to the charge nurse when necessary.
• Proactively offer assistance and comfort measures to patients and families and present a positive professional manner with sensitivity to others’ needs.
• Provide patient care under the direction of the RN, including hygiene, nutrition, elimination, physical comfort measures, and additional advanced skills (e.g., phlebotomy, EKG, removal of Foley catheters, removal of IVs).
• Assist RN and other health care team members with care delivery, direct patient admission, monitor data entry for key patient safety information, participate in charge reconciliation, and perform audits as requested.
• Support team initiatives, unit performance improvement projects, and professional nursing practice through shared leadership, evidence‑based practice, and ethical accountability.
• Transport unit supplies, patient care supplies, medications, and equipment; practice accountability and use effective written and oral communication with patients, staff, physicians, families, and visitors, following AIDET and ARRC guidelines.
• Document completion of patient care in the Electronic Medical Record (EMR) and communicate care concerns or emergencies to RN or Charge Nurse in a timely manner; use real‑time communication techniques.
• Maintain patient care equipment (blood pressure monitors, glucometers, etc.); if devices malfunction, sequester, tag, and report to appropriate personnel.
• Keep patient rooms and unit areas clean and free of clutter, follow PPE and infection control policies, enforce universal precautions, use proper body mechanics and assistive devices, and perform environmental of care rounds.
• Comply with all Wellstar Health System policies, standards of work, and code of conduct.
