Shift: Days/M-F (work outside of the normal business hours will likely be required of an employee in an exempt level position)
Pay Band: 11
Salary to be commensurate with qualifications of the selected candidate within the established range (generally minimum‑midpoint) of the position

Responsibilities- Assessments: complete assessments 30‑45 days prior to annual IPP conference, update data and action plans.
- Active treatment: interact/engage with individuals in living areas and on/off campus events.
- Training: complete in‑service and competency training with DBHDD and AU.
- Transition: maintain family preference information, meet with transition management, document in avatar.
- Quarterly and case‑management notes: submit quarterly notes by last day of the month, case‑management notes after meetings or contact.
- DMA‑7: forward DMA‑7 forms to nurse, scan and upload to chart.
- Individuals liaison: follow up on medical staff calls, obtain consent, assist with paperwork, handle visit requests.
- Purchasing/Discarding: procure furniture/equipment per protocol, inventory items.
- Final notes: record death summary and discharge summary in avatar.
- Skilled unit (if applicable): update MDS book monthly, attend QAPI monthly.
- Meetings: leadership meeting once a month, statewide DBHDD monthly, continued habitation weekly, human rights monthly.
- Legal issues: inform appropriate personnel of guardianship changes, assist legal paperwork, coordinate with sheriff’s office.
- Other duties as assigned.
Educational Requirements:
Master’s Degree in Social work, Psychology, or a relevant Social science field from an accredited institution and a minimum of 2 years of directly related experience.
- Ability to maintain confidentiality.
- Detail‑oriented.
- Excellent interpersonal, verbal, and written communication skills.
- Proficiency in Microsoft Office and other relevant software/databases.
- Successful background check and, if applicable, credit check.
- Compliance with vehicle reports for positions that require driving.
- Faculty hires must provide proof of academic credentials and, if applicable, credential evaluation.
- Adherence to cybersecurity and privacy policies.
