Job Summary- Must be a highly organized individual, as this position will require a significant amount of tracking and follow-up for many different job functions
- Requires completion of a significant number of Needs Assessments for new individuals as well as annual renewals for individuals on the Planning List
- Requires telephone support to individuals and families on the Planning List, and bringing resolution to phone calls and messages
- Requires reporting Planning List changes and concerns to Planning List Supervisor
- Make and resolve referrals to outside agencies for non-waiver services
- Other supports for Planning List as needed.
- Bachelor’s degree, Human Services field preferred.
- Experience working with individuals with intellectual/developmental disabilities is strongly preferred.
- Must have at least an intermediate level of computer knowledge, Office knowledge preferred.
- Must report to Region Two Field Office in Augusta, Georgia.
- Experience with or knowledge of individuals with intellectual/development disabilities, Medicaid waivers, and DBHDD I/DD services; experience working successfully without daily contact with a supervisor.
- Spanish fluency
