• Must be a highly organized individual, as this position will require a significant amount of tracking and follow-up for many different job functions
• Requires completion of a significant number of Needs Assessments for new individuals as well as annual renewals for individuals on the Planning List
• Requires telephone support to individuals and families on the Planning List, and bringing resolution to phone calls and messages
• Requires reporting Planning List changes and concerns to Planning List Supervisor
• Make and resolve referrals to outside agencies for non-waiver services
• Other supports for Planning List as needed.

• Bachelor’s degree, Human Services field preferred.
• Experience working with individuals with intellectual/developmental disabilities is strongly preferred.
• Must have at least an intermediate level of computer knowledge, Office knowledge preferred.
• Must report to Region Two Field Office in Augusta, Georgia.

• Experience with or knowledge of individuals with intellectual/development disabilities, Medicaid waivers, and DBHDD I/DD services; experience working successfully without daily contact with a supervisor.
• Spanish fluency
