** flexibility and a hybrid work environment**(home, community, office setting). We are looking for a Care Coordinator to join our team! The ideal candidate will have experience in providing care coordination services, including developing care plans, coordinating resources, and advocating for clients. The Care Coordinator will be responsible for monitoring client progress, ensuring that all services are provided in a timely manner, and working with clients to ensure their needs are met.

Responsibilities
  • Develop and implement individualized care plans for clients.
  • Educate clients on health and wellness topics.
  • Write the Plan of Care for each member.
  • Identify and link member with community resources.
  • Facilitate referrals and respond to social service needs.
  • Collaborate with other team members including doctors, nurses, other
  • Help members implement the Plan of Care.
  • Monitor and assist them in problem solving potential issues related to the health care system, financial or social barriers.
  • This roll will also support and encourage members in actively participating in reaching the goals they set together!

We are looking to hire one team member in the Augusta area and one in the Skowhegan area and one in the Richmond area.

Requirements
  • Excellent communication skills for effective interaction with clients and healthcare professionals.
  • Ability to work collaboratively within a team environment.
  • Organizational skills to manage multiple client cases efficiently.
  • MHRT/C, MHRT/Cp OR at least 1 year working as a Peer Support, Recovery Coach or in the Daily Living Skills field.
  • OR bachelors degree and experience working in mental health.
