Job DescriptionThe Home Health Case Manager Registered Nurse (RN) works with a multidisciplinary team to coordinate care, manage caseloads, and uphold high quality standards. The role provides flexibility in scheduling, autonomy in patient visits, and support from a triage team, an education department, and shared governance committees.
Work Responsibilities- Consult with nursing staff and the multidisciplinary team to evaluate the patient’s status and appropriateness of care, including admission, length of stay, transfer, and discharge.
- Provide direct treatment, education, and management of health conditions, recommend additional services and referrals for community resources.
- Plan, implement, and evaluate patient care plans based on assessment to optimize outcomes.
- Monitor, record, and communicate patient condition as appropriate.
- Active Maine RN license or ability to obtain the license in a timely manner.
- At least one year of acute or med/surg (or equivalent) nursing experience.
- Previous ED, ICU, CCU, or home care experience preferred.
- Valid driver’s license and acceptable driving record.
- Strong technology/computer skills and experience with electronic medical records (EMR) preferred.
32 or 40 hours per week (candidate’s choice at hire). Limited on‑call, weekend, and holiday rotation. Staff must be flexible to work in and travel to areas surrounding the South Team’s territory (Greater Augusta).
