Qualifications:

Active and unencumbered LPN license
Graduate from an accredited nursing program
Current BLS certification
Previous experience in a clinical or long-term care setting preferred
Excellent interpersonal and communication skills
Strong attention to detail and ability to follow care protocols
Ability to work collaboratively with physicians, RNs, and healthcare staff

Key Responsibilities:

Provide direct, day-to-day patient care and monitor patient health
Administer prescribed medications and document responses
Assist with patient assessments, vital signs, and routine procedures
Communicate changes in patient status to clinical team and families
Support care team with various clinical tasks as assigned
Maintain up-to-date, accurate records in compliance with facility policies
