Job Summary

Maine General Health is seeking a dedicated and proactive Registered Nurse (RN) Care Manager to join our team at Gardiner Family Medicine.

Schedule:

Full-time (40 hours/week), Shift: Days (Mon‑Fri)

• Interview and assess patients, family, caregivers, or legal representatives.
• Determine, prioritize, provide, or arrange needed internal and external services and interventions.
• Consult with multidisciplinary teams to evaluate patient status and appropriateness of medical care, including admission, length of stay, transfer, and discharge.
• Participate in discharge planning, coordinating patient transfers to other facilities and community resources.
• Provide discharge education and resource referrals to patients.
• Perform chart review to identify issues affecting service delivery, outcomes, satisfaction, compliance, cost, and reimbursement.

• Active Maine RN licensure or ability to obtain within a timely time frame.
• At least 1 year of RN experience preferred.
• Previous care‑management experience preferred.
• Excellent teamwork, communication, and collaboration skills.
• Proven critical‑thinking skills.
