Job Summary

The Nurse Navigator (NN) is a licensed registered nurse with disease‑specific clinical knowledge who offers individualized assistance to patients, families, and caregivers to help overcome barriers to care. Utilizing the nursing process, an NN provides education and resources to facilitate informed decision‑making and timely access to quality health and psychosocial care throughout all phases of the cancer continuum. The role is both patient‑and‑provider facing, interfacing in person, telephonically, and electronically, and is not a bedside position.

• Key Role/Responsibility 1: Serves as a subject‑matter expert in the field of navigation; improves patient care timeliness and access to care; collaborates with program leaders and administration to perform and evaluate data from community needs assessments; provides relationship‑based, patient‑centered care consistent with population‑specific characteristics; participates in growth and development activities with the navigation team; provides leadership in program needs; collaborates with the overall navigation team for cross‑coverage and subject‑matter expertise to coordinate high‑level care for all patients.
• Key Role/Responsibility 2: Performs the nursing process (assessment, diagnosis, identification of outcomes, planning, implementation, evaluation) using evidence‑based practice, analytical and critical thinking; coordinates delivery and documentation of safe, quality patient care that promotes a respectful, inclusive clinical environment; demonstrates teamwork and collaboration; supports performance improvement and shared governance; coordinates care as recommended at specialty clinics and multidisciplinary rounds; identifies gaps in services and locates resources to bridge them.
• Key Role/Responsibility 3: Completes all initial and ongoing professional competency assessments and mandatory education; serves as a preceptor or mentor for other nurses and staff; stays apprised of advances impacting cancer care by participating in professional opportunities; promotes evidence‑based practice, nursing research, and performance improvement; upholds all system policies and procedures, including legal, regulatory and accreditation requirements; participates in data collection and consultation for practice questions;
  
  supports efficient use of resources; performs other duties as assigned; complies with all Wellstar policies and code of conduct.

• Associates Nursing or Diploma (Nurse)
• Bachelors Nursing (Preferred)

• All certifications are required upon hire unless otherwise stated.
• BLS - Basic Life Support; ARC‑BLS – American Red Cross Basic Life Support; BLS‑I – Basic Life Support Instructor
• RN – Registered Nurse (Single State) or RN‑COMPACT – RN Multi‑state Compact

• Minimum 3 years nursing practice
• Minimum 2 years nursing with disease‑specific experience (Preferred)
• Outpatient care or care coordination experience (Preferred)

• Ability to function in a fast‑paced environment
• Can respond to emergencies in a decisive, composed, and respectful manner
• Possesses excellent time‑management skills
• Practices nursing using evidence and analytical skills along with strong critical‑thinking skills
