Registered Nurse Health Care Navigator
The incumbent is also responsible for coordinating transportation and lodging to support Veterans in accessing cardiovascular procedures in the nearest Hub VAMC that has procedural capabilities. The RN Healthcare Navigator reports to the CARDIO-VET Nurse Manager, however, is indirectly responsible to the CARDIO-VET Social Determinants of Health Program Coordinator for portions of their duties involving socioeconomic issues such as transportation, lodging and other associated needs.
Registered Nurse Health Care Navigator duties include but are not limited to the following:
- Assesses, plans, implements, and evaluates care based on age specific components.
- Assumes responsibility for the coordination of care focused on patient education, self-management, and customer satisfaction throughout the continuum of care.
- Follows procedures per established policies and guidelines.
- Influences care outcomes by collaborating with members of the interdisciplinary team.
- Executes position responsibilities that demonstrate leadership, experience, and creative approaches to management of complex client care.
- This position may require travel to multiple duty stations within the CARDIO-VET Network.
- Executes streamlined approach to receiving, triaging, and directing timely, coordinated care.
- Demonstrates expert practice and decision-making, deliberate planning, and critical thinking skills.
- The nurse navigator is assigned to Veterans requiring complex care coordination or specialized case management services.
- Serves as the primary point-of-contact to Veterans and their families or caregivers and communicates with the health care team while leading complex care coordination and case management efforts.
- The nurse navigator duties are multifaceted and may be divided between registered nurse functions of the Cardiology Hub and Spoke network which includes consult management, care management and coordination of benefits.
- Serves as a liaison for the Veteran between VA facilities and is responsible for managing the care of the Veteran throughout the process.
- Supports the Cardiology Hub and Spoke network's mission to improve timely access to cardiology care.
- Functions at their highest level of role performance and aligns high-quality care with key performance indicators to ensure business practice standards meet patient care needs.
- Works collaboratively as a member of the interdisciplinary team, makes caring for the Veteran their propriety while in the work setting, and demonstrates customer service principles in all aspects of work.
- Provides peers with informal constructive feedback for improvement.
- Supports colleagues and other nurses through knowledge sharing to provide safe, quality nursing care.
- Fosters a safe and supportive environment conducive to the professional development of healthcare professionals.
- Contributes professional nursing perspective in discussions with the interdisciplinary team.
- Partners with others to effect change and produce optimal outcomes.
- Evaluates outcomes of evidence-based decisions and practice changes for individuals, groups, and populations.
- Responsible for the documented outcomes at the program or service level.

Pay:
Competitive salary, regular salary increases, potential for performance awards

Work Schedule:
7:30am - 4:00pm, Monday - Friday. A compressed work schedule is available if it supports the agency mission. This position will travel infrequently by government vehicle to all Community Based Outpatient Clinics (CBOCs) and to the procedural hub. Travel is up to 25%.
