Job Overview
Full‑time Clinician (Nurse Practitioner or Physician Assistant) to provide in‑home health evaluations, engaging with patients in their home environment and bridging gaps in care. The role includes medication & medical history review, physical evaluation, and optional diagnostic tests such as diabetic eye exams and spirometry.

- Help visit millions of people nationwide, connecting them to the right care for their unique needs.
- Work collaboratively with Signify Health as a clinician colleague, fulfilling the requirements of the specified role.
- Be flexible to travel locally and within licensed states as needed.
- Conduct virtual visits as required.
- Provide all indicated Signify Health services, including Diagnostic & Preventive Services products such as spirometry, DEE, and bone density screening.
- Active, unrestricted license(s) in the coverage area(s).
- Board certification (required).
- Multi‑state licenses (preferred).
- Ability to cover the primary location:
Augusta, ME.
Typical pay range: $78,331.00 – $. Actual base salary depends on experience, education, geography, and other factors.
