RN Clinical Assessor – Waldo​/Kennebec

• Conduct person-centered medical and/or behavioral health assessments using standardized tools and criteria
• Escalate complex or non-standard cases outside defined protocols
• Conduct on-site, video, and/or telephonic assessments to determine service and support needs, eligibility, and level of care
• Seek guidance from senior staff as needed
• Perform duties in compliance with Person First standards, HIPAA guidelines, and company confidentiality policies
• Ensure accurate and consistent documentation of assessment outcomes
• Travel to client, consumer, or program locations for in-person assessments exceeding 50% of the workweek
• Evaluate individuals in homes, group homes, and care facilities to determine Long Term Care needs and appropriate placement options

• Specific clinical license and experience requirements based on contract requirements
• Minimum two (2) years of post-graduate nursing experience
• Ability to travel on-site to client, consumer, or program locations for more than 50% of each workweek
• Experience in long term care or home health care preferred
• Experience with OASIS or MDS preferred
• Reliable high-speed internet service: minimum 20 Mbps download/50 Mbps for shared internet connectivity and minimum 5 Mbps upload
• Private and secure workspace

Demonstrates expertise in conducting person-centered medical and behavioral health assessments while ensuring compliance with HIPAA guidelines and Person First standards. Proficient in evaluating long-term care needs and maintaining accurate documentation of assessment outcomes.
