You will lead nursing care per the Plan of Care across home, assisted living, or patient homes, collaborating with physicians, LPNs, CNAs, social workers, and chaplains to refine care plans.
You will perform initial and ongoing assessments, provide education to patients and families, and document findings while maintaining infection control and compliance standards.
